The form has to be filled in no more than 48hrs before you travel.

Go to the gov travel site there you can get the form and there is no cos

NO COST  you are on the wrong site if it’s asking for a fee. You can do it online or download the app

You need 

Passport no.     Flight no.   Seat no. Vacation date and type address where you are staying in spain

You should also take your proof of being double jabbed in case you get challenged.

You can download a copy using the NHS app, the downloaded copy has an expiry date.

Or, you can request a paper copy by dialling 119, this copy does not have an expiry date.

When I entered Spain yesterday via Malaga, they showed no interest in the NHS double jabbed certificate, but they did scan the QR code on the Spanish locator form.

Hope this helps.  I'll say this is correct at the time of writing the response but...

FOR TRAVEL FROM UK TO SPAIN

- PCR Test no later than 48hrs before you land in Spain (Need confirmation of negative result) we booked through Boots to get this done, you have to book the PCR test online and make a separate appointment for your actual test, also online. Once you’ve been tested you can log back onto the Boots test centre to check your results AND they email you a copy as well which can be used as your fit to fly certificate.

- Proof of vaccination, either use your NHS App to prove double jabbed, or

 print of the created .pdf from the app (Once printed this is valid for 30 days from the date printed). If you use the app this does not expire as it recreates a new QR code every time you open the app.

* I would however recommend printing a copy off for ease.

- Complete Spanish locator form, link to website here: https://www.spth.gob.es/. This can be partly completed any time before you travel, however you can only finally complete 48hrs prior to arrival in Spain. You need to answer the covid questions and validate your Vaccine’s. You will need to compete this as a ‘Family’ or ‘Individually’. Once you receive this by email, print this off for each of you and present once off the plane before collecting your luggage in the airport.

FOR TRAVEL FROM SPAIN TO UK

- Antigen test (This test is currently acceptable) as your 'Fit to fly' test, need to show this negative result at check-in desk. Either email confirmation or print out.

Link here to book your 5min test at Malaga airport, you need to book individual tests for all travelling regardless of Vaccine status. Make sure you select MALAGA Airport from the drop-down menu. https://travelcovidtestcenter.com/es/

   - Will also need proof of double jab, again use NHS app or print off (Valid for 30days from printing) again recommend the printed version as you can use this for both outbound and inbound. Again, if you printed it for your outbound trip and you haven’t stayed longer than 30 days, the same print out will be valid for your return journey.

- Complete UK locator form (This is checked when checking your passports at UK Immigration). Again, remember you can only complete this document

 48hrs before you are due to arrive back in the UK.

- I would recommend Ordering your Day 2 (Return) tests BEFORE you leave, they are usually delivered next day so save at home for your return. (At this point of writing) Order a Day 2 test from Randox or equivalent for each person. If you do purchase from Randox try code: TUI2021 this should give you a reduction. Purchasing this online will give you a confirmation email, this will also include unique reference numbers for each of the tests ordered, these need to be added to your UK passenger locator form, NOTE: You will not be able to complete your locator for if you have not done this step.

- Currently if your double jabbed no need to self-isolate in the UK unless your day 2 test comes back positive. Think that’s it, have a nice trip. ;-)

Just a point of note I did this help sheet for families so if you are double jabbed OR have children under the age of 12 you do not need to do the outbound (From Uk to Spain) PCR test. 👍🏻

And it’s 72 hours before - not 48 for pcr into soain
