My wife and I are both retired and obtained our residencias in October 2019, just before Brexit.
To get these we had to include S1 forms amongst our packages of evidence.
Although we’ve not registered with a doctor or health centre, if one of us became ill, would we still be entitled to free health care and can we apply for a Spanish EHIC card?

its two years since we did this so I don't remember the details but we had no problem submitting our S1 forms and after a little while we received our EHIC card. We then registered in our local health centre and the EHIC card allows you to collect prescriptions. Our experience to date had been quite positive with the Spanish public health system although it has been a strange couple time during the pandemic. I would definitelt recomment getting the card.
