I have a health card via S1 but never used, I am with Sanitas who are very good indeed, part of BUPA and a third of the cost UK. I'm not keen on the public clinics here and can have long wait times. Private health here very cheap, there are no restrictions on my private insurance, any meds I have just paid for at the Pharmacy, quick and simple. I'm aware could get percentage of meds paid, but means getting the prescription from the state clinic, which means a lot of time. Good luck getting your card,

Some replies sure have been confusing for you!

With residencia, we were able to register with the local public health service, using our EHIC card, but had to renew yearly.

With S1 you go permanently on to the public health service, which is the Spanish NHS. The attention received and the treatment received are both first class. I read an article last week about a survey of medical care in 18 developed countries, The UK came 17th overall, Spain came 4th.  

You do NOT pay anything to the doctor, your treatment is billed back to the UK. Same for hospital treatment.

The only thing you pay for is a small (I think 10%) percentage of your prescriptions.

Going to a private doctor has no connection at all to the public health service, I think one or some of the previous answers would confuse you on that. If you have the S1 and are put on the Spanish health system you definitely do not need to go private.

I've seen references to long wait times, so here is my recent experience. Toenail problem with possibilty of cancer. Saw the GP Thursday 12 noon. Seen by the consultant at the hospital 8.30 am the next morning.

I would not take above is the norm, sometimes long waits in medical centres and know people who have waiting a long time for things like scans, my Spanish neighbour had to wait for months. They envy the fast private service. perhaps they thought was an emergency, no idea,
