Thank you. Sanitas seems a good option, and I am familiar with S1. What is of concern for me is insurance cover for my pre-existing conditions. If I can receive with an S1 treatment for free in Spain, like the NHS in the UK. What are the medical facilities like in Nerja? Are they on a par with the UK's NHS?

However I’ve read that the Spanish government is hoping to persuade the EU to abolish this rule for British people living in Spain . So could be good news for you 

I doubt the rule will change, not possible due to Spain having no border and being part of Schengen, late in 2023 people will also need a electronic Visa called 'ETIAS', 'no longer stay' still '90 out of 180 days',, people will apply on line, note heading states May, but moved until November. .

I believe a non lucrative visa (NLV) will circumvent such rules, given that we are over 70 yrs of age.

A NLV will give you a TIE residence card, valid,at first for one year, stay then needs to be renewed, so yes no 90 day in 190 rule.

The TIE ID card is what you get with your NLV to stay longer than 90 days. The bad news , that makes people a Spanish resident for tax purposes, so need to get the right advice, the tax year here in Spain starts 1st January.
