Here you go. I tried online bank transfers from the UK for the first couple of years but find it easier to complete on line then print off and pay at a local collaborating bank when I am in Spain, in my case it's Banco Sabadell. It's fairly starightforward and (IMHO) I don't think it needs paying someone to fill out a basic form and hand it over to the bank. Message me privately if you need any more help with it. My understanding is that the form has to be submitted by the end of the calendar year following the year you are entering details for.
