CAN I GET THIS FROM PHARMACY WITH PROOF OF MY BOTTLES WITH NAME ETC ON IT PLUS THE OTHER HALF OF UK DISPENSE GREEN SLIP FOR YOUR CURRENT MEDICATION OR WILL A DOCTOR HAVE TO SIGN A PRESCRIPTION FORM

In a similar situation I asked my UK GP for a prescription before I left England. This I was able to take to a local pharmacist in Spain who dispensed it without a problem.
